Download & print the form
Download the PDF order form below and print one copy for each transformer you want to equip — every kit is custom-built to a specific unit.
Enter transformer details
Record the transformer unit/TC number, serial number, manufacturer, KVA rating, primary and secondary voltage, and the oil sample valve size.
Add your measurements
Use the diagram on the form to note the distance from the sample valve to where you want the exterior box mounted, and which side or face it should sit on.
Complete shipping & billing
Fill in the ship-to and bill-to company, contact, and address details so we can fabricate and deliver your kit.
Choose payment
Pay by credit card, check, money order, or purchase order. Payment is collected before the custom kit ships.

Email the completed form to rod@invz.net or contact our office. We review the specs and confirm your order.
